COVID-19 Vaccine Requirements
IMPORTANT: Everyone MUST WEAR a FACE MASK
ALL Applicants who are getting the Medical Exam from a Civil Surgeon for form I-693, ON or AFTER October 1, 2021 MUST complete the COVID-19 vaccine series.
Pfizer: 2 doses
Moderna: 2 doses
J & J: 1 dose
The COVID-19 vaccines can now be given at ANY TIME without regard to the timing of other vaccinations.
All children ages 5 – 17 years old can get Pfizer’s 2 dose vaccine. They are mandatory.
